2008 SUMMER CAMP FORMS

The forms are now available for boys and girls sport summer camps. Please make sure both pages are returned in full to the business office. Forms may be turned in during normal office hours or may be returned by mail to: Helias High School, 1305 Swifts Highway, Jefferson City, MO 65109.

The parent signature form must be turned in prior to the first day of camp.
